STANDARD FORMS AND HANDOUTS
The following is a list of different forms, flyers, brochures, booklets, and pamphlets available at the City of Pasadena Housing Department (CoPHD).
A Good Place to Live
Booklet helps persons to understand what the housing quality standards are and why they are important.
Affidavit Form
Form must be completed by family member who is making a certification. Affidavit must be notarized.
Affidavit Form – Self Employment
Form must be completed by a family member who is reporting self employment. Affidavit must be notarized.
Affordable and Workforce Rental Housing
Shows the units in Pasadena that are affordable.
Assumption Contract
Form is given to new owner of a property for execution, in order to assume the lease and contract already in place.
Authorization for the Release of Information Form – HUD
Form along with the HUD Authorization for the Release of Information must be signed by all adult family members. This form is usually provided along with the Personal Declaration form or the Waiting List Update of Family Circumstances form.
Authorization for the Release of Information Form – CoPHD
Form along with the HUD Authorization for the Release of Information must be signed by all adult family members. This form is usually provided along with the Personal Declaration form or the Waiting List Update of Family Circumstances form.
Child Support/Alimony Certifications Form
Form is completed by the participant or applicant.
Debts Owed to Public Housing Agencies and Terminations Form
Informs participants that HUD maintains a repository of debts owed to PHAs and adverse information of former participants who have voluntarily or involuntarily terminated participations in one of HUD Rental Assistance Programs.
Declaration of Section 214 Status Form
Form is provided at the time of a waiting list appointment. Each family member must have one completed by the actual person unless the family member is a minor. If the family member is a minor, then the legal guardian completes the form. Form is also provided for a participant on the program who is adding someone to their case.

Federal Privacy Act Statement Form
Form is provided at the time of a waiting list appointment.
Fraud
Fraud is generally defined in the law as an intentional misrepresentation of material existing fact made by one person to another with knowledge of its falsity and for the purpose of inducing the other person to act, and upon which the other person relies with resulting injury or damage. Fraud may also be made by an omission or purposeful failure to state material facts, which nondisclosure makes other statements misleading

Housing Assistance Payment Contract
Housing Choice Voucher Program Contract (Section 8) - find your own place and use the voucher to pay for all or part of the rent.
Housing Information Line
The City of Pasadena HOUSING Department automated voice service has changed to better serve the community. This 24/7 (24 by 7) service provides details on housing assistance, home buyer or ownership assistance and housing related services including owner & landlord inquiries.
Housing Opportunities for Persons with AIDS (HOPWA) Program

How to Find a Rental Unit

Lead-Based Paint Information
Form is provided at the time of the Briefing Session, regarding Lead –based Paint Poisoning.
Move with Continued Assistance - Policy and Procedures
Families already on the program who are interested in moving with assistance (need a new voucher) must follow the policy and procedures for moving.
Obligations of the Rental Assistance Program Participant Family Form
Form is provided to the family at the time of annual re-examination, briefing session, or when family is adding a family member.
Participant Fact Sheet
This form is mailed with the annual renewal packet to program participants.
PayMode
Bank of America direct deposit program for landlords to enroll in order to receive rental payment in the form of an electronic payment transfer into ones personal account.
Personal Declaration for Rental Assistance Benefits Form
Form is provided to family at the time of their annual re-examination, waiting list appointment or prior to leasing up.
Public Housing Authority Website Directory
Provides the name of the housing authorities and locations.
Request for Reasonable Accommodation Form
Form provided by the Housing Assistants only. Persons with Disabilities may request a reasonable accommodation.
Request for Voucher Extension Form
Voucher holder requesting an extension must submit this form.

Self-Certification Form
Form is completed by family member who is certifying to a statement made. Form does not need to be notarized.
Shelter Plus Care Program (S+C)
Hand out provides information on the Shelter Plus Care Program.
Statement of Policies
Form is including in the RFTA package, this form gives a brief summary of the CoPHD policies and procedures for the Rental Assistance Program.
Statement of Property Ownership Form
Form is completed by the property owner/authorized agent before a contract is executed. This form is also used by property owner/agent to report address changes or management changes.

Update of Family Circumstances Form
Families on the program (not applicants on the waiting list) reporting changes not at the time of the annual process must complete this form.
Waiting List Personal Declaration for Rental Assistance Form
Form is provided to family on waiting list prior to leasing up or at appointment.
What You Should Know About EIV Form
The Enterprise Income Verification (EIV) system is a web-based computer system that contains employment and income information of individuals who participate in HUD rental assistance programs. All Public Housing Agencies (PHAs) are required to use HUD’s EIV system.
